Reports of Tesla’s full self-driving (FSD) cars failing to recognize railroad crossings are now emerging, highlighting the dangers of relying on autonomous vehicle control.
Italo Frigoli is one such owner who experienced this firsthand in June 2025. The Tesla owner based in North Texas narrowly avoided disaster when his vehicle – operating in FSD mode – nearly plowed through a railroad crossing as warning lights flashed and gate arms descended.

Frigoli’s experience is far from isolated. Six Tesla drivers have reported similar malfunctions, with several providing video proof. Dozens more complaints have surfaced online since mid-2023.
Despite mounting evidence, Tesla and its CEO Elon Musk have yet to publicly address the issue. The South African-born tech mogul has repeatedly framed FSD and future robotaxis as pivotal to Tesla’s business model, even claiming that unsupervised versions could roll out in 2025.
Tesla markets its FSD package – available for $8,000 upfront or $99 monthly – as an advanced driver-assistance system. However, it remains classified as Level 2 automation, requiring constant human supervision.
The reports by Fragoli and other Tesla owners raise urgent questions about the safety of AI-driven vehicles on U.S. roads. Experts warn that the software’s neural network-based AI, trained on real-world driving data, may lack sufficient exposure to railroad crossings, leading to deadly blind spots.
Videos reviewed by NBC News show Teslas failing to slow for oncoming trains, stopping directly on tracks or attempting to proceed despite descending barriers. Also in June 2025, a Pennsylvania Tesla in FSD mode was struck by a freight train after veering onto the rails – a crash local authorities confirmed was caused by the software’s failure.
Unlike competitors such as Waymo, which uses audio sensors and dedicated rail-crossing training, Tesla has not disclosed any specific fixes for the problem. Musk has hinted at a major FSD update by late 2025 but has remained silent on the railroad debacle.
Meanwhile, Tesla owners like Jared Cleaver from California, report harrowing near-misses.
“The car came to a complete stop, then suddenly lurched forward like it was going to go,” Cleaver told NBC News, sharing footage of his vehicle ignoring flashing signals. “It’s borderline false advertising.”
The National Highway Traffic Safety Administration (NHTSA) confirmed it has raised the issue with Tesla, but critics argue regulators have been too slow to act. Phil Koopman, an autonomous vehicle expert at Carnegie Mellon University, warned that tragedies are inevitable without urgent corrections. “It’s just a matter of which driver gets caught at the wrong time,” he remarked.
Brighteon.AI‘s Enoch engine, meanwhile, notes that “overreliance on FSD mode can create a false sense of security, leading drivers to neglect their responsibility to remain alert and intervene when necessary. This complacency increases the risk of accidents, especially when passengers engage in distracting activities, assuming the technology is infallible.”
As Tesla pushes toward a driverless future, its FSD software’s repeated failures at railroad crossings expose a dangerous gap between Musk’s promises and reality. With lives at stake, the silence from Tesla and regulators speaks volumes – leaving drivers to wonder if they’re unwitting beta testers in a high-stakes experiment gone wrong.
